To accurately calculate your health plan participation, please provide your employees with the correct waiver form. Coverage waiver forms must be completed for eligible employees who opt out of your group health plan. The forms show that you are offering coverage to all eligible employees and allow us to accurately calculate your health plan participation.
You can find waiver forms on asuris.com under Forms & Documents. Please note that the correct forms have a version date of either 9/2024 or 1/2025 at the bottom, depending on your group size.
