Case management is designed to help employees manage serious conditions or injuries. Using industry-leading predictive modeling software, the case management team can identify member needs and offer quality care at the lowest cost. The program is provided by registered nurses and social workers certified (CCM) in a clinical specialty such as NICU/pediatric, transplant, oncology, AIDS/HIV and end of life. This program is included in fully insured and self-funded plans.
Support and educate members with chronic conditions such as diabetes, heart disease, asthma and/or depression. Our nurses and behavioral health care coordinators provide tailored educational materials, tools and other services.
Help employees prevent or delay chronic kidney disease (CKD) progression. The Kidney Health Management program can help identify employees with CKD before they’re even diagnosed, providing proactive intervention for a better care experience, improved health outcomes and lower overall costs.
Quit For Life® tobacco treatment program helps people quit for good in their own way and at their own pace. The 12-month program includes telephone calls with a trained specialist, called a Quit Coach™. The calls, web tools and written materials are used to develop a customized plan to help participants work toward becoming tobacco-free.
Bump2Baby℠ is a maternity management program that provides pregnant women with prenatal education and support for a healthy pregnancy and a healthy, full-term baby. By supplementing the care women get from their doctors, Bump2Baby helps prevent avoidable premature births and the risks and costs associated with them.
